Personal Data Processing Authorisation

For some of our services, we need to process some of your personal data (such as your name, contact details and address). You can formalise your authorisation by signing the form below and handing it in at our shop in person.

How to proceed

1. Download and print the form.
2. Fill in your details and tick the authorisations you wish to grant.
3. Sign it and hand it in at our shop, Rua de São Gonçalo 33B, 2925-787 Azeitão.

This form is only available in Portuguese, since it is meant to be handed in physically at our shop in Azeitão, Portugal.
